چکیده

Chronic obstructive pulmonary disease is prevalent condition commonly associated with cardiovascular diseases. When both are combined the prognosis of the patient worsens. One cornerstone therapy for most cardiac diseases is beta-blockade, however concerns about its potential harmful effects on airways function often restrains their use in patients with COPD and coexistent cardiac diseases. While selective beta1 adrenergic blockers seem to have a better safety profile, other non-selective beta-blockers can be securely utilized in stable COPD patients with no or little reactivity when they are indispensable and used with caution. Evidence provided by post hoc analysis of clinical trials and large observational studies suggests a beneficial effect of beta-blockers on mortality and exacerbations in mild to moderate COPD patients. Benefits are less obvious in severe COPD. Studies on the actual use of beta-blockers suggest that many cardiac patients with COPD (or vice versa) who can benefit from beta-blocker utilization do not receive such medication as it is recommended in the guidelines because of die-hard, not justified in most cases, concerns on safety.

Beta-blockers as single-agent therapy for hypertension and the risk of mortality among patients with chronic obstructive pulmonary disease.

PURPOSE To assess the association between the type of antihypertensive medication and all-cause mortality among patients with chronic obstructive pulmonary disease (COPD). METHODS The cohort comprised 1966 patients (mean [+/- SD] age, 65.8 +/- 10.7 years) enrolled in general internal medicine clinics at seven Veterans Affairs medical centers between December 1996 and October 1999. Patients ha...

Heart Rate and Cardiovascular Disease: An Alternative to Beta Blockers

Ivabradine, an I(f) inhibitor, acts primarily on the sinoatrial node and is used to reduce the heart rate with minimal effect on myocardial contractility, blood pressure, and intracardiac conduction. Heart rate reduction is an important aspect of care in patients with chronic stable angina and heart failure. Many patients with coronary artery disease have coexisting asthma or chronic obstructiv...

Beta-blockers use in patients with chronic obstructive pulmonary disease and concomitant cardiovascular conditions

The mortality and incidence of chronic obstructive pulmonary disease (COPD) and coronary heart disease increase with age. Despite the clear evidence of beta blockers (BBs) effectiveness, there is a general reluctance to use them in patients with COPD due to a perceived contraindication and fear of inducing adverse reactions and bronchspasm. BBs are well tolerated in patients with cardiac diseas...

Effect of beta-blockers on exacerbation rate and lung function in chronic obstructive pulmonary disease (COPD)

BACKGROUND Beta-blockers are commonly prescribed for patients with cardiovascular disease. Providers have been wary of treating chronic obstructive pulmonary disease (COPD) patients with beta-blockers due to concern for bronchospasm, but retrospective studies have shown that cardio-selective beta-blockers are safe in COPD and possibly beneficial. However, these benefits may reflect symptom impr...
